Thanks for your interest in bringing Affinity to your school!

Affinity equips young men from low opportunity circumstances with the mentors, experiences and tools needed to increase their confidence, graduate from high school, excel in college and thrive as adults.

Using a group mentoring approach, Affinity’s one school year program guides cohorts of ~15 young men in their junior year of high school through a curriculum that focuses on. both social emotional growth and college access. Each cohort of young men is matched with 4-5 adults mentors.After a year of our program we anticipate that our young men will increase their confidence, self-awareness, relationship skills and decision making skills. Our ultimate aim is that 80% of our program participants matriculate into a 4 year college.

The cohort meets once a month at a partner school for two hours during after school hours. Monthly in person meetings are supplemented with once per month 1 hour online check-ins with the cohort. After its pilot year, Affinity will reassess whether there should be more frequent check-ins throughout the month (either in person or virtually). Additionally, as the COVID-19 crisis continues to impact the United States, virtual meetings allow for Affinity to continue to serve young people. As this crisis continues to unfold, Affinity may either increase or decrease the number of in person sessions to adhere to any local, state and Federal guidelines.

Volunteer Application Form

The information you give will be used not only to determine your eligibility for this program, but also to link you to the student most appropriate given your background and interests. All data collected will be used only on a “need to know” basis. Pursuant to THE MINNEAPOLIS CITY DEPARTMENT of EDUCATION policy, this information will be used to clear you to work unsupervised with an individual student. It is required that you complete this form below and submit it to Affinity prior to the mentoring training sessions.

Begin with your most recent educational experience. List the name of the institution(s), years attended and the degree(s) received.
Begin with your most recent job. For each entry, include job type, your job title, and a brief description of your responsibilities. If you have a resume, you may email it to info@affinitymn.org
List current and past participation in community activities. Include any involvement with youth-serving agencies.
List the languages in which you are fluent.
List the names and e-mail OR mailing addresses of three persons who have known you for at least one year and well enough to vouch for your character. One of these persons must have been an employer or supervisor. None may be from your immediate family. References will only be contacted upon your conditional acceptance into the program. A sample of the letter that will be sent to your references is included in this packet, for your information. Please alert your references that someone from the Department of Education will be contacting them.
